Information for new and current patients

We are proud of our staff of physicians, nurses, receptionists and staff — delivering to you the highest quality care in the field of gastroenterology. Our facilities and state-of-the-art equipment help us provide you with comfort and safety, and we are dedicated to serving our patients with genuine care and respect.

On your first visit we will ask you to complete some personal and medical information. For your convenience, these forms can be downloaded below. Please bring the completed forms with you to your appointment.

Appointments

To make an appointment, call 901-761-3900 from 8am – 5pm, Monday through Friday. If you cannot keep your scheduled appointment, please give us at least 24-hours notice so we can reschedule you and give your original appointment to someone else. Three missed appointments and/or late cancellations may result in fees and/or separation from the clinic.

For appointments involving endoscopic procedures requiring sedation, you will be asked to arrange for someone to drive you home after your appointment, since you cannot legally drive or operate machinery.

Emergencies

One of our physicians is available for emergencies at all times. During office hours, please call our office and relay the information to the receptionist — we will respond promptly. For emergencies after office hours and on weekends, please call 901-761-3900 and press “0” when the recording begins to connect to our after hours answering service.

Questions

We realize you may have problems or questions between appointments. Please call us during office hours (8am – 5pm), our office staff will route the call to appointments, billing, our lab, or will refer the call to one of the nurses if the call is of a medical nature. If the nurse feels the matter needs to be addressed by your physician, they will return your call as soon as possible.

Prescriptions

For better care, please try to call as early in the day as possible for your refills. We do not prescribe new medications over the phone. No routine prescriptions will be filled at night or on weekends.

Test Results

One of our physicians will review your test results and then telephone you, usually within 2 weeks, or as the results become available.
